Appointment and consultation information may be reviewed by Dr. Mike or authorized practice personnel.
Submitting a form or scheduling a complimentary consultation does not, by itself, establish a doctor-patient relationship.
Please provide only the information requested. Do not use a scheduling form to send Social Security numbers, payment information, complete medical histories, urgent health concerns, or other highly sensitive information unless the form has been specifically designated for that purpose.
Health Clue Finder
The Health Clue Finder is an educational symptom-exploration tool supported by Lovable.
The tool may collect the symptoms, concerns, or educational topics a visitor selects. These selections are used to provide general educational information and possible topics to discuss with a qualified healthcare professional.
Unless clearly stated otherwise:
Health Clue Finder selections are not added to a patient medical record
